CHOICE Funding Opportunities
CHOICE is excited to offer funding opportunities to support community-driven efforts that address the health and social needs of our region. These investments are designed to strengthen local capacity, foster collaboration, and improve outcomes for people and communities.
Sponsorships
Through small sponsorships up to $1,000, we aim to invest in events and initiatives that support Medicaid-eligible people by strengthening community health, deepening social connection, and improving access to Medicaid services and outcomes across our region. This includes supporting groups or organizations that host events to expand opportunities or reduce barriers for Medicaid-eligible individuals, while also creating shared opportunities to promote CHOICE and reinforce our mission, values, and strategic priorities.

Request for Applications (RFA) – Catalytic, Time-Limited Investments to Expand Healthcare Services
The purpose of this funding opportunity is to provide catalytic, time-limited investments that launch or expand healthcare services designed to become fully sustained through Medicaid billing within six (6) months of implementation. This opportunity will place particular emphasis on improving healthcare access and outcomes, as well as strengthening referral pathways between funded partners and the Community Care Hub.
Approximately $1.5 million in funding is available to support efforts throughout the CHOICE region. Applications will be accepted from January 16, through February 20, 2026.
