This framework connects all the elements of the plan, the cohort, and the pilots under a unified purpose.

Health Equity Learning Cohort (2.0)

The learning cohort is a group of 19 organizations across the region representing different service sectors. The cohort shares learning, refines HEIP strategies, learns from and supports HE pilot projects. 

Community-Initiated Care Materials

This discussion document aims to expand understanding of CIC as a scalable, person-centered model that extends the reach of health systems by integrating trained community members into the continuum of care.
