Request for Information – Health Related Social Needs Investments
We are seeking input from partners across our region to help shape the future of Health-Related Social Needs (HRSN) services. CHOICE is seeking to invest approximately $3 million in infrastructure funding through June of 2028. Through this Request for Information (RFI), we are gathering general information about community capacity, experience, and ideas to strengthen how services like housing, nutrition, and transportation supports are delivered and connected. This application includes a core set of questions for all respondents, along with additional questions for organizations that indicate interest in serving as an HRSN service provider or partner. Responses will inform future planning, partnerships, and potential investments to build a more coordinated, equitable system of care. This RFI is the first step for agencies seeking funds to launch or expand Health Related Social Needs services. We encourage interested organizations to review information on this page and other resource materials and submit a response through the application link by July 10, 2026.

What are HRSNs?
Health Related Social Needs (HRSNs) refer to the social and economic needs people experience that affect their ability to maintain their health and well-being. As part of the Medicaid Transformation Project 2.0, our State can invest healthcare dollars in meeting these needs.
By addressing HRSNs, we can advance health equity and reduce health disparities, which are core to CHOICE’s mission. It can also reduce health care utilization and costs.

HRSN Resources
Medical Respite
Medical Respite Care Services are available for eligible clients as a new Medicaid benefit, pending resource and provider availability. For more information about the benefit, who is eligible, a directory of providers, or how to become a contracted provider of services, visit the Medical respite care page on the Washington State Health Care Authority website (button below). Interfaith Works, in partnership with Providence, MultiCare, and CHOICE, has developed a six-bed Medical Respite Program in Olympia to serve medically vulnerable people with the intent to leverage this new Medicaid benefit for sustainability and growth. Housing Services
Housing Transition Navigation and Rent / Temporary Housing services are available for eligible clients as a new Medicaid benefit, pending resource and provider availability. Housing Transition Navigation
*** Live as of April 1, 2026. Housing Transition Navigation Services These services aim to remove barriers to affordable housing, including transition costs and housing deposits to assist with identifying, coordinating, securing, or funding one-time services and modifications necessary to help a person establish a basic household. Transition Assistance Program (TAP) The state will leverage the existing TAP to administer housing transition navigation services to enrollees of the FCS Supportive Housing program. Rent / Temporary Housing
*** Live as of April 1, 2026. These services provide stable, independent living situations for individuals transitioning out of institutional care or congregate settings who are homeless or at risk of homelessness. Payment may cover rent and/or short-term temporary stays for up to six months. If you are an FCS provider, you will need to register with Spokane Housing Authority who will receive and process approved rental assistance payments at https://spokanehsp.corerelief.com/content2/access/register Relevant Frequently Asked Questions: 1. Who is eligible for HRSN Rental Assistance? / What are the eligibility requirements? Individuals who: - Are 18 or older - Live in WA - Have a need for six months (or less) of rental assistance - Receive housing supports (FCS-Supportive Housing and / or Support Employment) Qualify for FCS under these risk factors: - They are homeless or formerly homeless; AND - Have complex substance, behavioral health, and/or physical health disability needs requiring assistance; OR - Experience frequent or lengthy stays in an institutional setting (e.g. residential, skilled nursing, inpatient hospital, psychiatric institution, prison or jail) 2. How do I apply for HRSN Rental Assistance? An FCS provider must apply on behalf of a client. Here is a map of FCS providers: FCS Provider Map 3. What expenses are eligible to be paid with HRSN Rental Assistance? Currently this benefit is only available to pay for rental arrears. In the future this may expand to Six months of rental arrears, up to a maximum amount of 150% of Fair Markey Rent (FMR). If rent includes utilities, those will be covered. 4. Will HRSN Rental Assistance be paid to the FCS-SH enrollee? No, rental assistance is paid directly to the landlord. 5. If I have questions, who can I contact? Nutrition Supports
Nutrition support services are available for eligible clients as a new Medicaid benefit, pending resource and provider availability. In 2025 CHOICE completed a region-wide nutrition landscape assessment to identify community needs, existing resources, and persistent gaps. For more information, view the one-page report here. Home Accessibility, Remediation, Adaptation
