Improving health access, equity, and outcomes
CHOICE (Community Health Organization Improving Care and Equity) is a nonprofit organization driven by a mission to reshape health equity and a commitment to ensuring everyone has access to quality care, resources, and connections that make life better.
We believe that healthy communities are built on strong relationships, where individuals are connected to essential services and to each other.
We serve everyone in the central western Washington region, which extends from what is now known as South Puget Sound to the Columbia River and from the foothills of Mount Rainier and Mount St. Helens to the Pacific Ocean. This is the historical and present-day territory of eight Tribes and Tribal Nations and seven county jurisdictions.

Promoting health equity
At the heart of our organization is an unwavering commitment to health equity. Our goal is to ensure that everyone, regardless of their background or circumstance, has the opportunity to live their healthiest life. This means addressing disparities and championing fair and just strategies in the ways we support communities.

Improving access and outcomes
We collaborate with local healthcare providers, community partners, and Tribal health leaders to fund and support regional health improvement projects. Through these partnerships, we're actively improving healthcare access, outcomes, and systems of care.

Community-centered approach, always
Our strength lies in the people and partnerships in our region. We are driven by the belief that community voices, experiences, and expertise contain the solutions needed for local transformation and change.
Accountable Communities of Health (ACHs)
CHOICE is the Accountable Community of Health (ACH) serving the Central Western Washington region.

ACHs are regional organizations that work with their communities on health care and social needs-related projects and activities. Nine ACHs were established by the Health Care Authority to be neutral conveners, investors, and coordinating bodies between the health care delivery system and local communities. ACHs receive funds through the Medicaid Transformation Project (MTP) to transform health systems. We distribute these funds throughout the region to support local health improvement projects.
