Coalition of ACHs

Medicaid (Apple Health) and SNAP Guidebook for Supporting Clients Through Federal Benefit Changes

We want to share a valuable new resource developed by the Coalition of Accountable Communities of Health (CACH) to help frontline staff, community-based organizations, and service providers support individuals and families who may be impacted by upcoming federal changes to Medicaid (Apple Health) and SNAP benefits.

The Medicaid (Apple Health) and SNAP Guidebook was created to help the community-based workforce better understand and explain benefit changes, identify next steps, and connect community members with the assistance they need to maintain coverage and access to benefits.

How to use this guidebook:

Use the guidebook as a resource when questions about Medicaid and SNAP changes resulting from federal policy changes come up in client case management services or everyday conversations. It can help to:

  • Answer common questions about Medicaid (Apple Health) and SNAP changes using clear, accessible information
  • Help identify where to direct clients for application assistance, additional information, and benefit navigation support
  • Support informed conversations about benefit changes and when referrals may be needed
  • Serve as a trusted resource to share with community members, partners, and staff

While the guidebook is intended to be a practical starting point and does not replace individualized benefits assistance, it can help community-based organizations and frontline staff confidently support clients as changes are implemented. Please consider sharing this resource broadly with your partner networks.

Additional Quick View Messages

To complement the guidebook, we are also sharing two brief Quick Key Messages resources that provide a high-level overview of the changes and key actions that can be taken:

Provider Key Messages: For service providers, community-based organizations, and frontline staff.

Client Key Messages: Designed for community members and clients.

These key messages are intended to serve as an introduction to the changes, while the Guidebook provides more detailed information, guidance, and resources for supporting clients and answering questions.
