CHOICE is proud to announce the awardees of our recent Time-Limited Catalyst Funding to support whole community wellness!
We received so many incredible applications from around the region with ideas around building and expanding services to meet community needs – dental, pediatrics, wrap-around services, behavioral health, and clinical programs were proposed.
Thank you to all of our applicants for taking the time to complete this application process for the chance to be awarded funds to complete wellness projects and objectives.
Following careful evaluation focusing on clear and specific paths to sustainability after our initial investment and impact on the care hub clients we are serving, CHOICE is pleased to announce $1.6 million in investment in 10 projects across our region.
CHOICE (Community Health Organization Improving Care and Equity) is a nonprofit organization dedicated to promoting health equity, and improving healthcare access, outcomes, and systems of care. We collaborate with local healthcare providers, community partners, and Tribal health leaders to fund and support regional health improvement projects.
