At CHOICE, we’re driven by an unwavering belief that everyone, regardless of their background or circumstance, should have the opportunity to live their healthiest life. One of the ways we’re working towards this goal is through funding opportunities that prioritize access to care, health equity, and community collaboration and innovation.
Through requests for proposals (RFPs), grants, and contracts, we’re striving to make strategic investments in local solutions and strengthen connections between social services and healthcare delivery systems.
But we need your voice, expertise, and insight to help inform this work.
We are currently searching for volunteer panelists to review future applications for RFPs and other funding opportunities.
We encourage individuals who have experience in one or more of the following areas to sign up:
Health equity
Community-based care coordination
Community health workforce
Access to care
Respite care
Lived experience
If you would like to volunteer as a future reviewer, please complete this form. When review opportunities come up, we will contact interested individuals to confirm participation. Every review opportunity may differ in terms of timeline and commitment. If we contact you, we will share more information about the process so that you can decide whether to move forward.
We encourage you to share this with others in your community or networks who might be interested.
Contact us at info@crhn.org if you have questions.
