CHOICE is excited to announce the formation of the Hub Advisory Board – a dedicated group to help guide our Community Care Hub’s work in connecting people to essential health and social services.
Why join the Hub Advisory Board?
Our Community Care Hub provides vital support to individuals navigating complex systems of health and social services. Many people face fragmented, unclear paths when they seek the help they need, often finding it difficult to know where to turn. The Community Care Hub was created to change this: with a dedicated community-based worker supporting each person every step of the way, we aim to build a future where everyone has access to the resources they need – when they need them – to thrive.
To achieve this vision, we need insights from those who know the region best – people like you.
What we’re looking for in Hub Advisory Board members
We’re inviting community members, service providers, and local leaders across various sectors to apply. Individuals must reside within central western Washington, including the geographic boundaries of various counties and indigenous nations. These include Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston, and Wahkiakum counties, as well as the Chinook Indian Nation, Confederated Tribes of Chehalis, Cowlitz Indian Tribe, Nisqually Indian Tribe, Shoalwater Bay Indian Tribe, Skokomish Indian Tribe, Squaxin Island Tribe, and the Quinault Indian Nation.
Members should be:
Collaborative and committed to improving access to care and resources.
Passionate about health equity and removing barriers for underserved populations.
Engaged in the community, whether through direct service, policy, or advocacy.
Your voice will help shape our approach, ensuring we connect individuals to services effectively and build partnerships that support well-being across the region.
Members can expect to commit to meeting at least quarterly for two hours. There may be additional 1-5 hours of work to prepare for or implement action items outside of meetings.

Community impact: Work alongside the CHOICE team and other local organizations to nurture a coordinated network, ensuring resources are accessible when they’re needed.
For members participating who are not being paid by their employer to do so, CHOICE will follow Community Compensation Guidelines from the Office of Equity.

If you’re interested or know someone who would be a great fit, please complete this short Google form no later than December 13. We encourage all voices and backgrounds, especially those representing underserved communities, to apply. We anticipate selecting and notifying all applicants of their membership status by the end of January 2025, with the first meeting of the Hub Advisory Board occurring in March.
