Good afternoon, valued partners.  CHOICE is releasing this Request for Interest to contract with community and tribal partners that provide, or may want to provide, community-based care coordination services across *Central Western Washington.  We are interested in partnering with organizations and tribal nations that want to strengthen our Community Care Hub, which helps people access the services they need when they need them. Please click here for a description of our current Community Care Hub. We would especially like to partner with organizations who can provide social care coordination services for community members with unique experiences and needs. This includes providing services to community members: Â
