We are excited to announce that CHOICE Regional Health Network and Cascade Pacific Action Alliance have merged into a single Accountable Community of Health (ACH) entity, now known as CHOICE (Community Health Organization Improving Care and Equity).
While our organization evolves, our core mission remains the same. We are here to champion, support, and invest in regional projects that improve access to care, improve health outcomes, and reduce costs.
We serve everyone in the Central Western Washington region, which is home to unique and diverse communities in the jurisdictions of Thurston, Mason, Lewis, Cowlitz, Pacific, Grays Harbor, and Wahkiakum counties and the sovereign nations of the Chehalis, Cowlitz, Nisqually, Quinault, Shoalwater Bay, Skokomish, and Squaxin Island Tribes.
Continuing our work: We will continue our current initiatives, including our Community Hub, Medicaid Transformation Projects, suicide prevention, opioid response, local community convening, and Blue Zones Activations in Mason, Lewis, and Grays Harbor counties.
Community governance: Our new single board structure brings together public, private, non-profit, and Tribal representation to inform a unified community-driven direction for our organization.
Building a community hub: We’re committed to building a robust community Hub network that connects traditional healthcare with community-based services that address health-related social needs (HRSNs).
Our definition of health equity is simple yet profound: it means everyone can live their healthiest life. We believe that achieving this vision requires fair and just strategies that are driven by an unwavering dedication to the people and partnerships in our region.
