• Support partners to increase internal capacity and strengthen care coordination networks by investing in business practices, workforce development, technology, and community outreach and engagement.

• Development and coordination of Pharmacy Services

• Improving systems of care for Veterans

• Needs assessment and care coordination planning

• Staff to support expanding the continuum of services available and accessed, including integration of cultural identity and arts, wellness, and educational components

• Resource directory development and testing

• Electronic Health Record System implementation and customization, including closed-loop referral systems
